Note  A participant is an individual who takes part in a research study.

1) You must have a receipt for each participant that receives payment of monetary value (i.e., cash, check, gift certificate)

2) When all of the receipts have been turned in, you must reconcile them on the Participant Reconciliation form provided in this file. Arrange the participant receipts in alphabetical order for recording on the reconciliation form. If you have more than one receipt for a participant, group them together and record the total amount paid out to them. In doing so, you only have to list them one time on the form.

3) The following information should be provided on the Participant Reconciliation form:
a) SSN#
b) Name
c) Address
d) Amount Paid

4) Additional sheets of the Participant Reconciliation form can be added.
